Christmas Cactus

Hello! I hope that I can get some help! My boyfriends grandmother gave me a very old Christmas Cactus. It has been in their family for so long. I feel very special that she gave it to me. The thing is, I think it's starting to die slowly (if you touch it it falls apart). I believe the transition shocked it or something (my boyfriend put it in his trunk and it fell over, losing some soil, etc), because it was alive and well at his grandmothers house. It's wilting, and look so sad. I would love if someone could help me figure out how to save this great plant! ETA: Based of help on here, I re-potted the plant in better soil (mixed it with a bit of the old soil), and fertilized. I gave it some water, as well. I'm going to put it out in the sun during the day and bring it back in at night. I hope that it helps the plant come back to life.
