Improving soil and perennials

Still a new gardener here so my thanks in advance.

I live in zone 4a and I have many perennials such a bee balm, lavender, blanket flower and purple coneflower that are in their second year and doing quite well. However, I’ve noticed that some of my soil is poor.

My question is do I need to remove the perennials (after I cut them back) when I add organic matter to the soil and mix it (the soil and organic matter)up and then put them back? I imagine that they are heavily rooted. Or do I work the soil around the perennials and leave them where they are now?
